I've noticed that everyone feels a sense of accountability when solving problems. This creates a transparent environment that's open for input from any direction. For example, designers will give engineers feedback, engineers will give design feedback, and anyone can share thoughts on scope, direction, priority, etc.

Autonomy is one of our core values, it was one of the key reason I was attracted to Sequence, and everybody in the company embodies it. It does happen quite often.
